Kombinierte antithrombotische Therapie bei Patienten mit Vorhofflimmern und koronarer Stentimplantation

Einleitung

Etwa 5 – 10 % der Patienten mit koronarer Stentimplantation haben Vorhofflimmern. Bei ihnen ist eine Antikoagulation indiziert, um das Risiko thrombembolischer Komplikationen (z. B. Schlaganfall) zu reduzieren. Bei koronarer Stentimplantation ist allerdings auch eine duale antithrombozytäre Behandlung notwendig, um die gefürchtete Stentthrombose zu verhindern. Die kombinierte, sog. antithrombotische Triple-Therapie schützt effektiv, ihr Nutzen muss sich aber mit dem jeweiligen Blutungsrisiko die Waage halten. Dieser Artikel beschreibt, wie – basierend auf einer individuellen, patientenbezogenen Risikoanalyse – eine fein abgestimmte Strategieplanung für diese Patienten erfolgen kann.
