Zusammenfassung

Studienziel:

Vergleich der Inklinationsveränderung der Unterkieferinzisivi durch die Kombinationsbehandlung von Herbstscharnier und einer vollständig individuellen lingualen Apparatur mit der Inklination des therapeutischen Set-ups. Zusätzlich: Überprüfung einer möglichen Veränderung der Neigung der Okklusionsebene nach Behandlung.

Methode:

Überlagerung von eingescannten Modellen zu den Zeitpunkten Behandlungsbeginn und -ende sowie virtuellen Set-up-Modellen von 12 Patienten in ein entsprechendes Fernröntgenseitenbild. In die Modelle waren eine Okklusionsebene und eine Frontzahnachse eingearbeitet, die nach Einfügen der Modelle in das FRS nachgezeichnet und zur Mandibularbasisebene vermessen wurden. Die durchschnittliche Behandlungsdauer mit dem Herbstscharnier betrug 9 Monate.

Ergebnisse:

Für 7 Patienten wurde eine Proklinierung, für 5 Patienten wurde eine Reklinierung der Unterkieferinzisivi geplant und auch erzielt. Die Veränderung der Neigung der Okklusionsebene zur Mandibularbasisebene durch die Kombinationsbehandlung betrug durchschnittlich 2,8°.

Schlussfolgerung:

Das therapeutische Set-up für die Herstellung einer vollständig individuellen lingualen Apparatur in Kombination mit dem Herbstscharnier kann mit einer Genauigkeit von 3° bezüglich der Inklination der Unterkieferinzisivi umgesetzt werden.

Abstract

Aim:

Evaluation of dental effects in the mandible by treatment with the combined Herbst and completely customized lingual appliance. In addition, examination of a possible variation of the inclination of the occlusal plane after treatment was done.

Method:

Digital models (time points: beginning and end of treatment as well as target set-up) of 12 patients were transferred in corresponding lateral headfilms. The models included an occlusal plane and a front tooth axis, which were traced and measured to the mandibular plane in the lateral head films. Average treatment time was 9 months.

Results:

For 7 patients incisor proclination had been planned and achieved and for 5 patients incisior uprighting had planned and achieved. Changes in the decline of the occlusal plane were on average 2.8° after the combined Herbst and completely customized lingual appliance.

Conclusion:

The planned (target set-up) and the final tooth position of the lower incisors correlated within 3.0° by using the combined Herbst and completely customized lingual appliance.
