Zusammenfassung

Erhöhter Blutdruck, gesteigerte Aktivität des sympathischen Nervensystems und des Renin-Angiotensin-Aldosteron-Systems, oxidativer Stress und Inflammation sind bedeutsam in der Pathogenese einer hypertensiven Herzkrankheit.

Die transthorakale Echokardiografie ist die Methode der Wahl zur Diagnose und Verlaufsbeobachtung der unterschiedlichen funktionellen und morphologischen Veränderungen des Herzens bei HHK.

Eine Koronarangiografie schließt eine koronare Herzerkrankung mit hoher Sicherheit aus. Nicht invasive nuklearkardiologische Verfahren erfassen die myokardiale Perfusion auf zellulärer Ebene und spiegeln die gesamte Perfusionsstrecke (Makro- und Mikrozirkulation) wieder.

Konsequente nicht medikamentöse und medikamentöse Therapien führen zur Normotonie und zum positiven Remodeling des Herzens bei HHK.

Abstract

Elevated blood pressure, increased activity of the sympathetic nervous system and the renin-angiotensin-aldosteron system, as well as inflammation are important factors in the pathogenesis of hypertensive heart disease.

Transthoracic echocardiography is the method of choice in the diagnosis and follow-up of functional and morphological cardiac changes in hypertensive heart disease.

Coronary angiography safely excludes the presence of coronary heart disease. By means of noninvasive molecular imaging myocardial perfusion at cellular level as well as the complete macrocirculation and microcirculation are visible.

Consistent drug therapy and lifestyle modification lead to blood pressure control and positive cardiac remodelling in patients with hypertensive heart disease.
