Zusammenfassung

Arzneimittel werden bei älteren Menschen sehr häufig kombiniert eingesetzt. Aber: keine Wirkung ohne Nebenwirkung. Daher ist es erforderlich, nach dem Ansetzen eines Medikaments genau hinzusehen und zu beurteilen: Wirkt das Medikament überhaupt und welche Nebenwirkungen sind aufgetreten? Genau diese Beurteilung wird oftmals nicht systematisch durchgeführt. Das Pilotprojekt NetzWerkPlan© zeigt auf, wie die Arzneimittelsicherheit in Pflegeheimen erhöht werden kann.
