Zusammenfassung

Die dreidimensionale kontrastunterstützte MR-Angiographie (3D-CE-MRA) mit paramagnetischen Kontrastmitteln hat die Möglichkeit eröffnet, hochqualitative nichtinvasive Angiographien in nahezu allen arteriellen Gefäßstromgebieten ohne Verwendung potentiell nephrotoxischer jodhaltiger Kontrastmittel in kurzer Aufnahme- und Auswertezeit zu generieren.

Die Methode eignet sich hervorragend bei Patienten mit Kontraindikationen gegen jodhaltige Kontrastmittel, wird aber auch zunehmend in der Routinediagnostik bei arterieller Verschlußkrankheit zur Festlegung der weiteren Therapie eingesetzt. Die Darstellung der Pulmonalarterien in Atemanhaltetechnik erlaubt die Beurteilung der Gefäße bis auf Subsegmentebene, so daß bei Verdacht auf Lungenembolien bei unklarem szintigraphischen Befund und Kontraindikationen zur Spiral-Computertomographie mit Röntgenkontrastmitteln eine exzellente nichtinvasive Methode alternativ zur Verfügung steht. Weiterentwicklungen mit Optimierung des Gefäßkontrastes durch neue Kontrastmittel und der räumlichen Auflösung durch verbesserte Spulentechnologie lassen eine zunehmende klinische Bedeutung der 3D-CE-MRA erwarten.

Summary

Threedimensional contrast-enhanced magnetic resonance angiography (3D-CE-MRA) with paramagnetic contrast agents allows rapid noninvasive work-up of arterial diseases with high diagnostic image quality without use of potentially nephrotoxic iodine containing contrast media. The method is therefore an appealing application in patients with contraindications for intraarterial angiography or spiral computed tomography with use of iodinated contrast agents. Due to the excellent image contrast 3D-CE-MRA is increasingly used as a routine procedure in the diagnostic work-up of peripheral occlusive arterial disease prior to further treatment planning. Breath-hold imaging allows high-quality examinations of the pulmonary arteries as an alternative to spiral computed tomography in patients with suspected pulmonary emboli and intermediate probability on lung ventilation/perfusion scans. Ongoing developments in contrast agents and surface coils aim at further optimizing image quality by increasing vessel contrast and resolution. 3D-CE-MRA is expected to become a feasable tool in the first-line diagnostic work-up of arterial disease in many applications.
