Zusammenfassung

Es wird über 239 Patienten berichtet, die in den Jahren 1954 bis 1959 wegen tumorbedingter Pleura- oder Peritonealergüsse intrakavitär Radiogoldkolloid erhalten hatten. Ein gutes Palliativergebnis wurde bei 48% der Fälle erzielt. Der Sitz des Primärtumors oder sein histologisches Bild waren ohne Einfluß auf den Behandlungserfolg. Ergüsse, die durch Lymphogranulomatose bedingt waren, zeigten keinen Behandlungserfolg. Die besten Ergebnisse wurden bei Mamma-Karzinomen erzielt, die jedoch auch den längsten Krankheitsverlauf vor Behand-lungsbeginn aufwiesen. Bei unbefriedigendem Erfolg einer ersten Behandlung sind die Erfolgsaussichten für eine zweite Behandlung ebenso groß wie diejenigen der Erstbehandlung bei einem bisher unbehandelten Patienten. Nebenwirkungen wie Nausea oder Durchfälle waren leicht und von kurzer Dauer. An ernsthaften Komplikationen wurde eine Myelopathie, 4 vorübergehende Leukopenien und ein Pleuraempyem beobachtet. Die günstigen Ergebnisse der Behand-lung lassen ihre Fortsetzung auch unter den erschwerten Bedingungen der heute gültigen Strahlenschutzvorschrift angezeigt erscheinen.

Summary

Results are reported of intracavitary therapy with colloidal radiogold for malignant pleural or peritoneal effusions in 239 patients during the years 1954 to 1959. Good palliative results were obtained in 48% of the cases. The location of the primary tumours or its histology were without influence on the results of therapy. Effusions which were caused by lymphogranulomatosis, did not respond. The best results were seen in breast cancer; however, they were the group with the longest course of the disease prior to treatment. In cases with unsatisfactory response to the first therapy the chances for a good response to a second course were the same as those for a good response to a first course in previously untreated patients. Side effects such as nausea or diarrhoea were slight and of short duration. Serious complications were one case of myelopathy, four transient leukopenias and one pleural empyema. The favourable results of this type of treatment appear to justify its continuation even under present-day difficulties caused by the new radiation protection regulations.

Résumé

239 malades ont été traités entre 1954 et 1959 par l’or colloidal radioactif pour des épanchements causés par des tumeurs. Un bon résultat palliatif a été obtenu en 48%. La localisation de la tumeur primitive ou son image histologique n’avaient pas d’influence sur l’effet thérapeutique. Les résultats les plus favorables pouvaient être obtenus dans les cancers mammaires. Ces derniers avaient le cours le plus long avant le traitement. Des épanchements causés par la maladie de Hodgkin ne prouvaient pas de réponse. En cas d’un résultat non satisfaisant du premier traitement, les chances thérapeutiques d’un second traitement sont aussi bonnes que dans un cas non traité auparavant. Des complications legères comme nausée et diarrhoe étaient passagères. Comme complication sévère on n’observait q’une myelopathie, quatre leucopénies et un empyème de la plèvre. Envisageant les résultats favorables, cette forme de thérapie nous semble justifiée, malgré les problèmes de protection s’imposant par les règlements de protection contre les rayons.
