Summary

The objectives of this observational study were to investigate whether varicography has additional value to CFDI in clarifying the nature and source of recurrent varicose veins below the knee after varicose vein surgery and to investigate the possible role of incompetent perforating veins (IPV) in these recurrent varicose veins. Patients, material, methods: 24 limbs (21 patients) were included. All patients were assessed by a preoperative clinical examination and CFDI (colour flow duplex imaging). Re-evaluation (clinical and CFDI) was done two years after surgery and varicography was performed. Primary endpoint of the study was the varicographic pattern of these visible varicose veins. Secondary endpoint was the connection between these varicose veins and incompetent perforating veins. Results: In 18 limbs (75%) the varicose veins were part of a network, in six limbs (25%) the varicose vein appeared to be a solitary vein. In three limbs (12.5%) an incompetent sapheno-femoral junction was found on CFDI and on varicography in the same patients. In 10 limbs (41%) the varicose veins showed a connection with the persistent below knee GSV on varicography. In nine of these 10 limbs CFDI also showed reflux of this below knee GSV. In four limbs (16%) the varicose veins showed a connection with the small saphenous vein (SSV). In three limbs this reflux was dtected with CFDI after surgery. An IPV was found to be the proximal point of the varicose vein in six limbs (25%) and half of these IPV were detected with CFDI as well. Conclusion: Varicography has less value than CFDI in detecting the source of reflux in patients with recurrent varicose veins after surgery, except in a few cases where IPV are suspected to play a role and CFDI is unable to detect these IPV.

Zusammenfassung

Das Ziel dieser Studie bestand darin, zu untersuchen, ob eine Varikographie im Vergleich zum Farbdoppler zur Ermittlung von Art und Ursache von rezidivierenden Varizen unterhalb des Knies nach Varizenoperation von zusätzlichem Nutzen ist sowie die Bedeutung insuffizienter Venae perforantes (IPV) für rezidivierende Varizen. Patienten, Material, Methodik: Die Studie bezieht sich auf 24 Beine (21 Patienten). Alle Patienten wurden einer klinisch und mit Farbdoppler untersucht: präoperativ und zwei Jahre postoperativ, zusätzlich mit Varikographie. Der primäre Endpunkt war das varikographische Muster dieser sichtbaren Varizen, der sekundäre Endpunkt der Zusammenhang zwischen diesen Varizen und IPV. Ergebnisse: Die Varizen waren bei 18 Beinen (75%) Teil eines Netzwerks, bei sechs Beinen (25%) schien es sich um eine einzelne Vene zu handeln. Bei drei Beinen (12,5%) fand sich im Farbdoppler und in der Varikographie beim selben Patienten ein insuffizienter Übergang von der Vena saphena magna zur Vena femoralis. Bei 10 Beinen (41%) zeigte sich varikographisch ein Zusammenhang zwischen Varizen und der persistierenden Vena saphena magna unterhalb des Knies. Bei neun dieser 10 Beine zeigte sich zudem im Farbdoppler ein Reflux dieser Vena saphena magna unterhalb des Knies. Bei vier Beinen (16%) bestand ein Zusammenhang zwischen den Varizen und der Vena saphena parva. Bei drei Beinen wurde dieser Reflux nach der Operation mit Farbdoppler nachgewiesen. Eine IPV fand sich als proximaler Punkt der Varizen bei sechs Beinen (25%), die Hälfte dieser IPV wurde ebenso mit Farbdoppler nachgewiesen. Schlussfolgerung: Die Varikographie ist zum Nachweis der Refluxursache bei rezidivierenden Varizen nach Operation von geringerem Nutzen als der Farbdoppler, außer in wenigen Fällen, wenn der Verdacht besteht, dass IPV eine Rolle spielen und mit Farbdoppler nicht nachweisbar sind.

Résumé

Le but de cette observation est de chercher à savoir si la phlébographie présente un intérêt complémentaire à l'échographie couleur (EC) pour l'étude et l'origine des varices récidivantes post-opératoires en-dessous du genou et de savoir quel est le rôle des veines perforantes incontinentes (VPI) dans ce type de récidive variqueuse. Patients, matériel, méthode: 24 membres inférieurs (21 patients) ont été investigués. Tous ont bénéficié d'un examen clinique pré-opératoire par EC. Un deuxième examen (clinique et EC) a été fait deux ans après un traitement chirurgical avec en plus une phlébographie. Le but principal de ce travail a été l'étude topographique des veines variqueuses visibles. Le deuxième but a été de chercher la relation entre ces veines variqueuses et les VPI. Résultats: Dans 18 jambes (75%) les veines variqueuses formaient une sorte de réseau; dans 6 jambes (25%) les veines variqueuses ont semblé isolées. Dans 3 jambes (12,5) a été mise en évidence une incontinence de la jonction saphéno-fémorale par EC ainsi que par phlébographie. Dans 10 jambes (41%) les veines variqueuses ont montré une relation avec une veine grande saphène (VGS) à la phlébographie. Dans 9 de ces 10 jambes l'examen par EC a montré également un reflux de la VGS en-dessous du genou. Dans 4 jambes (16%) la veine variqueuse a montré une communication avec la veine petite saphène (VPS). Dans 3 jambes ce reflux a été détecté par EC après traitement chirurgical. Dans 6 jambes (25%) l'origine proximale de la veine variqueuse s'est révélée être une VPI et la moitié de celles-ci a été détectée par EC de manière tout aussi bonne. Conclusions: La phlébographie a moins d'intérêt que l'EC dans la recherche de l'origine d'un reflux chez les patients présentant des veines variqueuses récidivantes après chirurgie à l'exception de quelques cas dans lesquels une VPI était suspectée de jouer un rôle et que l'EC n'a pas pu la mettre en évidence.
