Summary

Radioisotope renograms were obtained in the upright and prone position in 9 normal subjects, in 5 patients with untreated essential hypertension and in 21 hypertensives under treatment, showing moderate postural hypotension.

No significant renographic change were seen in the two positions in normal subjects and untreated hypertensives. Treated hypertensives with postural hypotension showed significant impairment of renal function in the upright position in 15 cases and no change in 6. Renal creatinine clearance was lower in the group that showed renographic changes. Renography in the upright position is suggested as a convenient test for early diagnosis and follow-up of the adverse effects of antihypertensive treatment.

Des renographies ont été effectuées en position verticale et en position horizontale chez 9 sujets normaux, chez 5 malades avec hypertension essentielle nontraitée et chez 21 malades hypertensifs en traitement, montrant une hypotension modérée orthostatique.

Le premier et le deuxième groupe examinés ne présentaient aucun changement apréciable rénographique dans les positions en question. Chez les 21 malades avec hypertension traitée et hypotension orthostatique, cependant, la radiographie effectuée en position verticale a révélé, dans 15 cas, des troubles signifiants de la fonction rénale. La clearance de la créatinine rénale était plus basse dans le groupe qui montrait des changements rénographiques. La rénographie, effectuée en position verticale, semble être une méthode convenable pour le diagnostic précoce et la surveillance des effects adverses du traitement antihypertensif.

Bei 9 gesunden Personen, bei 5 Patienten mit unbehandeltem essentiellem Hochdruck und bei 21 behandelten Hochdruckpatienten mit mäßigem Druckabfall in aufrechter Körperhaltung wurden Radioisotopennephrogramme durch-geführt.

Bei den gesunden und den unbehandelten Patienten wurden keine signifikanten Veränderungen des Nephrogramms in der liegenden und stehenden Position beobachtet. 15 von 21 behandelten Hochdruckpatienten mit Druckabfall im Stehen zeigten eine deutliche Verschlechterung der Nierenfunktion. Die Nephrographie im Stehen ist als ein bequemer Test zur Frühdiagnose und der Beobachtung von ungünstigen Wirkungen der Hochdrucktherapie anzusehen.
