Zusammenfassung

Wunschfantasien oder Streben nach eigener Körperbehinderung sind als sexuelle Deviation (Apotemnophilie) bekannt. Diese Zuordnung wird jedoch den eigenen Zustandsbeschreibungen jener Betroffenen nicht gerecht, die mit dem Wunsch nach Amputation gesunder Körperteile die Herstellung eines ihnen entsprechenden Selbstbildes verbinden. Anhand eines Fallberichtes wird gezeigt, dass eine deskriptive Beschreibung der Symptomatik unter Verwendung aktueller Diagnosemanuale nur unzureichend möglich ist und ätiologische Gesichtspunkte stärkere Berücksichtigung finden sollten.

Summary

Fantasies about or the desire of being disabled are known as paraphilia (apotemnophilia). There are unsolved questions of classification in cases where there are no sexual connotations but rather a strong sense of wrong embodiment. We are presenting a case report to discuss classification difficulties with the current descriptive diagnostic manuals. An additional etiological classification could be helpful. Before establishing new syndrome entities in descriptive terms more research in the rather complex field of body identity disorders is required.
