Examination Relation Polyposis nasi, Asthma and ASA intolerance with Asthma Control Test (ACT)

Background:

Patients with chronic rhinosinusitis accompanied by polyp formation (CRSwNP) often coexist with asthma and ASA intolerance. The aim of the study was to investigate the rhinological and pneumological parameters of CRSwNP patients with ASA intolerance (CRSwNP-AAI) or without ASA intolerance (CRSwNP-A) in uncontrolled or controlled asthma.

Method:

Prospective Study, Patient Groups: 1. CRSwNP-AAI ACT ≥20 (4 m, 10w, ø 41y) 2. CRSwNP-AAI ACT < 20 m (4 m, 10w, ø 39y), 3. CRSwNP-A ACT ≥20 (8 m, 6w, ø 53y), 4. CRSWNP-A ACT < 20 (11 m, 3w, ø 52h) and 5. CRSwNP (6 m, 8w, ø 43y). The asthma control test (ACT) defined patient's asthma control: ACT score ≥20 controlled asthma; ACT score < 20 uncontrolled asthma. Acquisition of the following parameters: nasal polyposis score, FEV-1 and rhinological (VAS scores, RSOM-31) and pneumological questionnaires (AQLQ).

Results:

The NP score was not different significantly. The FEV-1 values of the CRSwNP-AAI group (ACT score ≥20 and ACT score < 20) were significantly lower compared to the CRSwNP group. The VAS scores for posterior rhinorrhea and facial pressure were significantly increased in CRSwNP-AAI (ACT < 20) compared to patients with CRSwNP-AAI (ACT score ≥20). Also, patients with CRSwNP-A (ACT < 20) scored the facial pressure significantly higher than patients with CRSwNP-A (ACT ≥20). The total scores of RSOM-31 and AQLQ were significantly increased in uncontrolled asthma patients (CRSwNP-AAI and CRSwNP-A ACT < 20) compared to controlled asthma patients (CRSwNP-AAI and CRSwNP-A ACT> 20).

Conclusion:

Uncontrolled asthma is associated with increased pneumological and rhinological complaints in CRSwNP patients +/- ASA intolerance.
