Introduction:

The management of the adult cleft nose is particularly difficult. Among the key challenges are the correction of the basal tilt of the nose and the projection of the nasal tip.

Methods:

We present a retrospective chart review to analyse a cohort of 12 patients who underwent correction of the cleft deformity with endonasal placement of enhanced alar strut and foundation grafts.

Results:

The outcome was analysed in an objective and subjective fashion. Both, nasal breathing and nasal form improved substantially. No relevant complications occurred.

Conclusion:

The concept of endonasal placement of a foundation graft proves to be well suited for correction of the adult cleft nasal deformity. Important advances and adjustments to the technique have shown to be of value and will be presented in detail.
