Zusammenfassung

Die Inzidenz manifester Thromboembolien beim Tumorpatienten liegt bei 5-15%. Jede ätiologisch unklare Thrombembolie muß Anlaß zur Tumorsuche geben. Pathophysiologisch kann als zentraler Mechanismus eine Kombination aus einer Gerinnungsaktivität durch Tumoroder auch Monozyten/Makrophagen-Prokoagulanzien und einer Akute-Phase-Reaktion angenommen werden. Im Rahmen einer Tumortherapie (Operation, Zytostatika- und Strahlentherapie) kommt es zur Akzeleration der Gerinnungsstörung, welche die klinisch eindeutig nachgewiesene erhöhte Thromboseinzidenz in dieser Phase erklärt.

Therapeutische Maßnahmen werfen spezielle Probleme auf, da einerseits eine Resistenz gegen Antikoagulanzien und andererseits aber auch eine erhöhte Blutungsneigung besteht. Bei malignen Thrombozytenvermehrungen besteht sowohl eine Thromboseals auch eine Blutungsgefahr. Für die biologische Relevanz der Aktivierung des Hämostasesystems bei der Tumorerkrankung sprechen der Nachweis von Fibrin unmittelbar an vitalen Tumorzellen, die direkte mitogene Wirkung von Gerinnungsfaktoren auf Tumorzellen und die Beeinflussung der Tumorprogression im experimentellen System und in der klinischen Medizin durch antithrombotische Medikamente.
