In autumn 1978 the worldwide first successful allogenic tracheal transplantation in man was performed by Rose KG and Sesterhenn K in the Dept. of ORL, Head & Neck Surgery, University Hospital of Cologne/Germany. The recipient was a 19 yo man suffering from a severe and long segment tracheal stenosis. After a long-term stay in hospital (9 weeks) the patient was discharged. He was able to return to his job as a craftsman. There were no further complaints concerning the respiratory system or the upper aero-digestive tract in the further course. In November 2018 a control examination including endoscopy of the trachea was performed, 40 years after the transplantation. In this contribution the clinical findings are presented. Furthermore an scientific overview of tracheal transplantation in man is presented.
