Prognostic significance of perioperative blood transfusions in head and neck squamous cell carcinoma

Aim:

To evaluate the prognostic impact of perioperative blood transfusions (PBT) for patients with head and neck squamous cell carinomas (HNSCC).

Method:

Retrospective analysis of survival of 354 HNSCC patients primarily treated with surgery with regard to preoperative anemia and PBT.

Results:

Patients who received PBT showed a reduced overall survival. An increase of cancer recurrences was not observed. PBT was needed more frequently by older patients in worse physical conditions and with advanced stage carcinomas. Subgroup analyses revealed, however, a comparable effect for younger patients, patients with p16-positive HNSCC and earyl stage cancers.

Conclusion:

Despite again confirmed, profoundly negative effects of preoperative anemia on disease free and overall survival we suggest a restrictive transfusion policy whenever possible.
