Hearing preservation and speech perception – experiences with the slim modiolar array CI532 (Cochlear®)

Introduction:

The slim modiolar electrode array CI532 is a preshaped, thin, atraumatic electrode which is placed in the scala tympani. The array is hold straight prior to insertion by a special sheath. Our center is well experienced and has been using this electrode array for many years now. The aim of this study was to examine hearing preservation and speech perception of our CI532 patients.

Methods:

128 adults with postlingual profound hearing loss (154 Implants) who were implanted between 10/2015 and 08/2018 could be included. All implantations were performed by the same surgeon. Preoperatively, at the first fitting, after 3 and 6 month the following parameters were assessed: pure tone audiometry, speech perception (Freiburger monosyllable test) and modificated Skarzynski hearing preservation score (SHP-Score, 125 Hz-4 kHz). The position of the array was controlled intra- and postoperatively by x-ray and a flat panel CT-Scan.

Results:

Postoperatively the mean hearing threshold (4F-PTA) decreased by 12.7dB. The median speech perception was at 15% after the first fitting, after 3 months at 50% and after 6 months at 55%. The modificated Skarzynski hearing preservation score (median) reached 60% at the first fitting but decreased after 3 and 6 months to 40%. The CT-Scan showed a scala tympani placement in 100%. A tip fold over was found in 5 cases (3.3%) and 3 of them could be repositioned intraoperatively.

Conclusion:

The CI532 offers an option to implant structure preserving. Hearing preservation and good speech perception directly postoperative are possible. In review with the literature our data show less tip fold overs. A special surgical approach and a high level of training and experience might be reasons for that.
