Challenges in the endoscopic treatment of nasal and paranasal sinus tumors

Introduction:

Nasal and paranasal tumors have the wide variety of heterogenic types, non-specific “benign” growth and severe local complications. Those tumors have an insidious onset, non-recognizable symptoms which often cause a delay in diagnosis and progress of the disease. When advanced disease stage is combined with the complex anatomy of this region the endoscopic management of those tumors becomes very questionable.

Methods:

We present the endoscopic surgery of selected cases of advanced tumors – two cases of angiofibromas in IIA-B, rare case of osteoma in frontoethmoidal recess with intracranial complication and esthesioneuroblastoma stage Kadish C.

Results:

All of the cases were successfull, without major complications. The challenges that we met in the management of advanced sinonasal benignancies and malignancies, were to reach negative resection margins, low risk of haemorrhagic incidents and avoidance of intracranial postoperative complication.

Conclusion:

Over the past couple of decades, the endonasal endoscopic sinus surgery has become a more effective approach for treatment of sinonasal tumors ensuring the patient's safety and non disturbing quality of life, with curative intent and reduced surgical morbidity. The usage of techniques of endonasal endoscopic resections give us the possibilities to evaluate the advantages of the approach and highlights the multi-disciplinary management of patients with these complicated local lesions.
