Intracellular staphylococci in chronic sinusitis - serologic detection

Chronic rhinosinusitis (CRS) is triggered by a number factors that are only partially detected so far. As intracellular staphylococci promote infective attacks, pre-operative verification were helpful for planning further therapies. Until today no relevant data concerning recognition of patterns in CRS patients are available.

Aim of our study was the serologic detection of IgE against staphylococcal toxins (SE) within the scope of allergologic examination of CRS patients. In serum specimen of 120 patients with chronic sinusitis IgE against SE A, SE B, SE C and SE toxic shock syndrome toxin (TSST) were determined and compared with total IgE levels, ECP, tryptase and IgE against further areoallergens (AA).

2 specimen (18.3 %) showed IgE values of at least 0.35 kU/L against one SE. Most frequent IgE prevalence was caused by TSST (68.2 %), followed by SE C (36.4 %), SE B (31.8 %) and SE A (27.3 %). Significant findings were seen in elevated total IgE values in 77.3 %. ECP was elevated in 36.4 %, tryptase only in 9.1 %. 40.9 showed sensitizations against AA. In case of IgE against SE 86.4 % had elevates total IgE or ECP levels (p < 0.01).

According to literature data in every second patient intracellular staphylococci can histopthologically be detected, but also in every third healthy subject. Long-term data show intermittent promotion of infection by SE with infiltration of inflammatory cells in the nasal mucosa. Our data show significantly increased comorbidity of elevated IgE and ECP values with IgE agaist SE. In terms of a prospective study clinical relevance is to be elucidated in regard of therapeutic options.
