Using FNAB in the diagnostic of the resistance in the parotid region – retrospective analysis of 651 patients

Objective Preoperative diagnostic management of lesions in parotid region is difficult due to wide heterogenity of tumors. Biopsy from the tumor is recommended step in diagnostic algorhm. Using FNAB (fine needle aspiration biopsy) as a basic examination method helps modify the scope of surgical procedure. We report our ten-year experiance with performing preoperative FNAB.

Methods The retrospective analysis of 651 patients. In all these patients was between years 2006 – 2016 performed FNAB preoperatively and the cytological results were compared with final histopatological diagnoses. The study included 367 females and 284 males, average age was 58 years. Cytologic results were classified to be positive (suspicious of malignancy), negative and non-diagnostic. The sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), accuracy and likelihood ratio were stated.

Results In 604 (92,8 %) patients was FNAB diagnostic, in 47 (7,2 %) patients was non-diagnostic. In 89 patients the cytological result was evaluated like positive (suspicion of malignant tumor). 515 FNAB results were negative (benign). Sensitivity was 80 %, specificity 93,82 %, PPV 62,92 %, NPV 97,28 %, LR positive and negative were 12,95 (0,21 respectively) and the accuracy was 92,22 %.

Conclusion Our results shows important role of FNAB in diagnostic algorithm of parodic gland masses.
