Ballontibioplastie zur Reposition von Impressionsfrakturen des Tibiakopfs

Zusammenfassung

Bei der Behandlung von Impressionsfrakturen des lateralen Tibiaplateaus gilt die offene Reposition und Osteosynthese als Methode der Wahl. Für ausgewählte Frakturen, bei denen die Impression der Gelenkfläche die Hauptkomponente darstellt, kann die Rekonstruktion des Plateaus nach der hier vorgestellten Methode jedoch auch minimalinvasiv mithilfe eines Kyphoplastieballons erfolgen, der statt konventioneller Instrumente zum Anheben der Gelenkfläche benutzt wird. Der Erfolg der Operation hängt dabei von der korrekten Indikationsstellung sowie von der Beachtung einiger Tricks und Fallstricke ab. Eigene Erfahrungen zeigen sehr gute postoperative Ergebnisse nach Tibioplastie von ausgewählten Frakturen.

Summary

Open reduction and osteosynthesis is the gold standard for the treatment of displaced impression fractures of the lateral tibial plateau. For selected fractures, where the impression of the articular surface is the main component, the reconstruction of the plateau can also be carried out minimally invasive using a kyphoplasty balloon, which is used instead of conventional instruments to elevate the articular surface. The success of the procedure depends on the correct indication, as well as on specific tricks and pitfalls. We have had very good postoperative results after balloon tibioplasty of selected fractures.
