Establishing multispectral optoacoustic tomography (MSOT) as in vivo diagnostics for mimic muscles

A peripheral facial palsy (FP) results in a deefferentation of the mimic muscles (MM). This in turn causes a denervation atrophy (DA). Sonography of MM is able to show volume changes of MM during FP. The aim is to establish multispectral optoacoustic tomography (MSOT) for in vivo diagnostic in mimic muscles. The effect of functional electrostimulation (FES) on DA should be examined by MSOT.

MSOT is a high-resolution optical method that can examine in vivo the hemoglobin-, fat- and collagen content of muscles. This has been shown already in skeletal muscles with muscular atrophy Duchenne. Since October 2021 MSOT is being established at ENT department Jena.

The study is planned as a monocentric prospective analysis in patients with FP who don´t show any signs of reinnervation in facial nerve electromyography. Grading of FP is done by Sunnybrook Facial Grading System (SGFS). Photos and 3D-videos will be taken. Quality of life will be documented by using Facial Clinimetric Evaluation (FaCE) and Facial Disability Index (FDI). DA will be examined by sonography and – for the first time- by MSOT. Two times a day patients will do FES for 20 min each until reinnervation, maximum for one year. Reevaluation of all shown parameters will be done every four weeks.

On the one hand, we expect short term changes in MSOT by rising oxygenated hemoglobin content during FES. Those changes can be shown graphic and quantitative.

On the other hand, we expect middle term changes in MSOT by influencing collagen content while using FES. First results of our study will be presented in Hannover.
