Indication and extent of a paratracheal neck dissection in differentiated thyroid carcinoma

The incidence of differentiated thyroid carcinomas has been increasing over the last few decades, although mortality remains almost unchanged. According to the literature, however, the incidence of aggressive forms, which are differentiated according to the histological findings, is increasing. This is one of the important reasons why an indication or extension of a paratracheal neck dissection must be carefully and individually made. Even if differentiated thyroid carcinomas usually have a good prognosis, patients with aggressive variants, which are difficult to identify preoperatively, benefit significantly from elective paratracheal neck dissection. Nevertheless, a paratracheal neck dissection brings with it risks, such as damage to the recurrent laryngeal nerve or hypoparathyroidism.
