Atypical cause of nasal obstruction: a rare case of primary hodgkin lymphoma of thenasopharynx

Background Nasal obstruction is a common complaint in the otolaryngology clinic with a wild differential diagnosis. Primary nasopharyngeal Hodgkin’s lymphoma is a rare cause nasopharyngeal tumors which can present as nasal obstruction.
Case report we present the case of a 45 years old male patient presented with history of nasal obstruction which worsened over the past months. Endoscopic nasal examination revealed a nasopharyngeal mass filling almost all of the nasopharynx. Computer tomography showed a well-defined mass filling the nasopharynx.
Results Transnasal excisional biopsy was made. Histopathology finding showed classical Hodgkin’s lymphoma. Immunohistochemical analysis confirmed the diagnosis. Full body PET-CT scan demonstrated activity in the nasopharynx and a right level II lymph node. No evidence of lymphoma in other part of the body. The patient was referred to onco- hematology where he was given chemotherapy and radiation. Control PET scan within 12 months with no active infiltration of Hodgkin’s lymphoma.
Conclusion Hodgkin’s lymphoma of the nasopharynx is a very rare cancer of the nasopharynx with only few reported cases in the literature. However, it should be considered in the differential diagnosis of the nasal obstruction caused by nasopharyngeal tumors.
