Speech development after cochlear implantation and cytomegalovirus infection
In addition to sensorineural hearing loss, newborns with cytomegalovirus (CMV) infection are at increased risk for additional disabilities of varying severity. This analysis concerns with language acquisition after cochlear implantation (CI) and CMV. The retrospective evaluation includes 32 children after CI, diagnosed with CMV and their results (n=15) in the German Language Acquisition Test for Four- Five Year Old Children (SETK 4-5) in the subtests, Sentence Comprehension (SV), Morphological Rule Formation (MR) and Sentence Memory (SG) and the most recently measured speech perceptive performance (n=21) in the Freiburg Speech Intelligibility Test and the HSM Sentence Test 10 dB SNR. Some Children could not be tested with SETK 4-5, because family language is not German (n=4), due to severe cognitive impairment (n=9) or because children did not cooperate (n=4).
Nine Children achieved age-appropriate results in SV, 5 children in MR and 6 children in SG. A strong correlation to later results in HSM 10 dB SNR is found between performance in SV (r=0.507) and in SG (r=0.502). Performance in MR and later scores on the Freiburg Speech Intelligibility Test correlate also strongly (r=0.594). Twenty-four children, including 13 children with additional developmental difficulties, use spoken language for communication. Children without active language acquisition all have severe multiple cognitive disabilities. Language acquisition after CI and CMV depends on the severity of the comorbidities. Language acquisition after CI and CMV is also possible for children with additional developmental difficulties. The results in SETK 4-5 can be helpful to consider inclusion in a mainstream school.
