Neue Therapieoptionen bei Autoimmunerkrankungen

Die Pharmakotherapie rheumatischer und anderer entzündlicher Erkrankungen hat sich seit der Einführung der Biologika wesentlich gewandelt. Etanercept und weitere Biologika zählen heute zu den erfolgreichsten Innovationen und bieten neben einem günstigen Nutzen-Risiko-Verhältnis auch die realistische Aussicht auf eine Remission. Dieser Beitrag stellt neueste Entwicklungen bei den Biologika, Biosimilars und den Small Molecules vor.

Kernaussagen
  • Biologika nehmen einen bedeutsamen Platz bei der Therapie zahlreicher Autoimmunerkrankungen ein, die mit konventionellen Therapien nicht ausreichend kontrolliert werden können.

  • Die bereits schon große Gruppe der Biologika erweist sich in nunmehr fast 20-jähriger klinischer Praxis als ein erheblicher Gewinn an therapeutischer Effektivität für eine Vielzahl entzündlicher Erkrankungen.

  • Der Einsatz von Biologika bei Erkrankungen mit Arthritis, Psoriasis, chronisch entzündlicher Darmerkrankung oder beim SLE muss differenziert erfolgen.

  • Bei der systemischen JIA sind vor allem IL-1- und IL-6-Hemmer einzusetzen. Bei der Polyarthritis TNF- und IL-6-Hemmer, bei der Psoriasis und der Psoriasis-Arthritis TNF-, IL-17- und IL-12/23-Hemmer.

  • Monoklonale TNF-Antikörper sind bei Arthritis, Psoriasis, Uveitis und entzündlichen Darmerkrankungen wirksam. Das TNF-Rezeptor-Fusionsmolekül Etanercept hingegen nur bei Arthritis und Psoriasis.

  • Abatacept ist bislang nur als ein Reservemedikament einsetzbar.

  • Nach Erreichen des Therapieziels (inaktive Erkrankung/Remission) ist ein Therapierückzug gegen die Rezidivgefahr abzuwägen und kann aus probatorischer Therapieunterbrechung, geringerer Dosierung und verlängertem Applikationsintervall bestehen.

  • Biosimilars erhöhen den Wettbewerb. Zudem profitieren auch Patienten von ihnen, denen zuvor eine Therapie mit dem Referenzarzneimittel vorenthalten wurde.

  • Die Inhibition der JAK/STAT-Signalkaskade – die Targeted Therapy mit Small Molecules – stellt einen aktuellen, innovativen Therapieansatz dar. Vorteile sind u. a. eine mit Biologika vergleichbar hohe Wirksamkeit bei oraler Applikation.

  • Die Hemmung der JAK/STAT-Signaltransduktion ermöglicht die Beeinflussung auch von zytokinvermittelter Entzündung, für die bislang keine Biologika verfügbar sind.
