Zusammenfassung

Das Lambert-Eaton-Myasthenische-Syndrom (LEMS) ist eine sehr seltene neurologische Autoimmunerkrankung, für die eine proximal betonte Muskelschwäche charakteristisch ist. Die Erkrankung ist als paraneoplastisches neurologisches Symptom bekannt, das besonders mit neuroendokrinen Tumoren assoziiert ist und v. a. beim kleinzelligen Lungenkarzinom auftreten kann. Das Merkelzell-Karzinom (MCC) ist eine seltene Malignität an der Haut, die eine Tendenz für lokale Rezidive und regionale Lymphknotenmetastasen hat. Es tritt v. a. bei älteren Patienten an den sonnenexponierten Hautarealen als schnell wachsendes intrakutanes Knötchen auf. Hier stellen wir einen Fall von MCC vor, das bei einem vergleichsweise jungen 40-jährigen Patienten mit LEMS und Vitiligo diagnostiziert wurde. Nach einer Überprüfung der Literatur zeigt sich das Auftreten von LEMS mit MCC und Vitiligo als eine Rarität.

Abstract

Lambert-Eaton-myasthenic syndrome (LEMS) is a very rare neurological autoimmune disease characterized by a proximal muscle weakness. The disease can often occur as a paraneoplastic neurological syndrome associated with neuroendocrine malignancies, esp. with a small cell lung carcinoma. The Merkel cell carcinoma (MCC) is a rare malignancy on the skin, which has a tendency for local recurrence and regional lymph node metastases. It occurs especially in older patients on the sun-exposed skin areas as a fast-growing intracutaneous nodule. Here, we present a case of MCC diagnosed in a comparably young 40-year-old male patient with LEMS and vitiligo. After a review of the literature, the simultaneous appearance of LEMS with MCC and vitiligo is an interesting rarity.
