Emerging evidence from multiple highly specialized groups continues to support a role for resection of the medial wall of the cavernous sinus when invaded by functional pituitary adenomas to offer durable biochemical remission. We present a case of a 25-year-old male with acromegaly and radiographic appearance of a KNOSP grade I adenoma with intraoperative visualization of medial wall invasion. The video demonstrates key steps in resection of the medial wall safely for sustained postoperative remission.
