Indications und limitations of the active hearing implant VibrantSoundBridge over time
Introduction Active middle ear implants provide an option for hearing rehabilitation in many cases, where conventional surgery is not successful. We present our data regarding the indications, limitations and outcome of the VibrantSoundBridge system (VSB) over time.
Methods Retrospective chart study, single-subject repeated-measures design in a tertiary referral center. All patients, implanted with a VSB device, were included. The follow up had to be at least 24 months. Audilogic measurements and medical records were evaluated, regarding medical problems and outcome. Air and bone conduction threshold and hearing improvement scores in aided and unaided condition were measured. Results During the last 12 years, 55 patients received 72 implants. The indications were malformations (n= 19), chronic otitis media (n = 42) or chronic otitis externa (n = 11). Nineteen patients were bilaterally implanted. The follow up varied between 24 and 120 months (average 66 months). The individual hearing improvement and stability over time depended on primary the primary indication and inner ear threshold at the point of implantation. Surgical revisions were indicated due to medical problems, e.g. cholesteatoma or extrusion, or inner ear deterioration. Only one case of technical malfunction of the implant was observed.
Conclusion VSB is approved for use in children and adults and provide an excellent option for hearing rehabilitation in patients with malformations or chronic otitis. According to our experience, the surgical procedure is safe and reliable. However, medical problems over time could force revision surgery or explantation of the device.
