Continuous intracochlear ECochG measurement to preserve residual hearing in CI surgeries
Introduction In order to preserve residual hearing during CI insertions, it is desirable to perform intraoperative monitoring of the cochlea. A promising approach is the measurement of cochlear microphone potentials (CM) using electrochleography (ECochG).
Methods During the insertion of hearing preservation CI electrodes, the potentials were derived intracochlearly via the CI electrode on contact 1. After insertion, different electrode contacts were used for recording. The stimulation was done acoustically using 500 Hz tone bursts and a special recording software from the company MED-EL was used for the recording, which enables a continuous measurement of the potentials during the insertion. So far, 4 patients have been included in the study.
Results In the cases measured so far, a high level of agreement between the measured amplitudes during and after insertion was found. There also seems to be a good connection to the later retention of residual hearing, but this needs to be further investigated.
Conclusion In principle, the continuous intracochlear ECochG measurement works and seems to provide consistent results on residual hearing preservation, but the patient group must be significantly enlarged for further conclusions.
