Transtympanic cortisone injections: Evaluation of clinical data of one year

Background Transtympanic administration of steroids is part of the therapy for acute hearing loss. High-dose corticosteroid administration is still recommended. Transtympanal injections are recommended especially in cases of AI due to underlying diseases/side effects.

Material and methods For hygienic and standardized application, 0.5ml pre-filled syringes are prepared with: Dexa 100mg Inject Jenapharm+Hylo Comod AT. Within one year, prefilled syringes were used in 69 patients. Lidocaine spray or anesderm ointment was used as the method of anesthesia.

Results In 52% of cases, oral corticosteroid therapy was initiated within 5.5days after the event. Glucocorticoid transtympanal administration was performed in 54 hearing loss, 1 blast trauma, 1 IED injury, 1 barotrauma, and 5 tinnitus patients. In a total of 16 female and 47 male patients, the average age was 54years, with the youngest patient being 21 and the oldest patient 81 years old. Before starting ITI, hearing loss averaged 70dB, with the highest hearing loss at 110dB and the lowest at 15dB. The mean hearing gain was 20dB, the maximum hearing gain was 75dB and the max hearing deterioration was 45dB. The response rate was 85% in our patient population. Relevant NW were 2persistent tympanic membrane perforations, 1 permanent taste disturbance and in 2 cases hearing deterioration.

Conclusions With a response rate of 85% and a hearing improvement of max. 70dB and on average at 20dB, transtympanic glucocorticoid injection is a treatment that should not be withheld from patients. Nevertheless, because of side effects requiring treatment in 5patients in a relatively small patient population, detailed education is needed.
