Transmastoid Approach to Repair Iatrogenic Cerebrospinal Fluid Otorhinorrhea After Cerebellopontine Angle Tumor Resection: Experience and Outcomes
Objective To evaluate surgical results and follow-up experience of transmastoid approach to repair Cerebrospinal fluid (CSF) otorhinorrhea after cerebellopontine angle (CPA) tumor resection.
Methods Three cases of iatrogenic CSF otorhinorrhea after CPA tumor resection from 2020 to 2021 were retrospectively analyzed. Three patients who underwent CPA tumors resection through retrosigmoid approach returned to the hospital due to CSF otorhinorrhea. After failure of conservative treatment or retrosigmoid surgical repair failed, abdominal fat was used to seal the mastoid cavity through the mastoid approach to repair cerebrospinal fluid leakage.
Results The iatrogenic CSF otorhinorrhea was successfully closed in all patients using the technique described. The patients were followed up for at least 10 months, and there was no recurrence of CSF otorhinorrhea or complications.
Conclusion Conservative treatments and retrosigmoid surgical methods for iatrogenic CSF otorhinorrhea are usually ineffective and can easily delay treatment, Especially for patients with well-developed temporal bone, transmastoid closure using autologous fat is a simple, effective, and safe option to repair CSF leaks.
