Results of multimodal Therapy of Squamouscellcarcinoma of the nasal Sinuses with Infiltration of the Orbita

Introduction Squamous cell carcinomas (PEC) of the nasal cavity (NHH) and paranasal sinuses (NNH) are rare tumors. An overarching growth to the orbit is reported in almost half of the cases and represents an interdisciplinary challenge.

Methods 49 patients presented for the first time in the period from 2006 to 2020 with a PEC of the NHH or NNH. Survival of tumors with and without orbital involvement was analyzed retrospectively.

Results The mean age at diagnosis was 65.5 (±10.7) years (78.4%m, 28.6%f). The origin was mostly in the maxillary sinus (48.9%) or in the NHH (40.4%). Spreading growth to the orbit was observed in 40.8% of cases (n=20). Orbital symptoms, such as visual loss or bulbus dislocation, are indicative of orbital involvement but are absent in two-thirds of cases. 47 patients were treated primarily curatively and surgery with adjuvant radiotherapy or radiochemotherapy was the most important (n=29, 61.7%). With orbital involvement, open surgical procedures and reconstructive surgeries were significantly more common. Three quarters of all operated patients with orbital involvement received reconstructive surcical intervention. Recurrences occurred in 34.7% (n=17), with no difference being found between the group with and without orbital involvement. The overall 5-year survival rate is 51%. The 5-JüR with orbital participation of 30% differs significantly from that without orbital participation (65.5%, p= 0.013 after log-rank test).

Conclusion PECs of NHH and NNH are rare tumors for which surgical therapy with reconstruction is the focus. Orbital involvement is common and an important prognostic factor.
