Thyroglossal duct cysts are the most common neck tumors in childhood. They arise ontogenetically due to the lack of closure or involution of the thyroglossal duct. Thyroglossal duct cysts can contain scattered thyroid tissue, which in very rare cases result in thyroid malignancies. In the three presented cases, the patients presented with an increasing mass prelaryngeally that had existed for several months. Clinically and image morphologically, the finding of a thyroglossal duct cysts was similar. Histologically, however, portions of papillary thyroid carcinoma were found. Due to the rarity of the described appearance of the tumor standards of thyroid cancer therapy need to be modified to this location.
