Spectrum of Primary Bone Tumors of the Talus: A Single Institutional Experience, 2021–2022

Purpose or Learning Objective: (1) To describe the radiologic features of the primary bone tumors of the talus; and (2) to demonstrate a multimodality approach for the diagnosis of primary bone tumors of the talus.

Methods or Background: Primary bone tumors of the talus are extremely uncommon, and only a few case series and case reports are available in the literature. The rarity of talus tumors often results in a delayed diagnosis. Surgical management also poses significant challenges because of the complex anatomy of the region. We describe our experience of the diagnosis and management of various primary tumors of the talus seen during the last 2 years.

Results or Findings: We searched our database retrospectively for all histologically proven primary tumors of the talus discussed by the multidisciplinary team and managed between 2021 and 2022. We analyzed the clinical presentations, radiographic appearances, and magnetic resonance imaging features of 18 cases and correlated our clinicoradiologic findings with the histopathologic and immunohistochemistry diagnoses. Histopathologically confirmed cases were described as giant cell tumor, 6; osteoid osteoma, 2; aneurysmal bone cyst, 2; desmoplastic fibroma, 1; osteoblastoma, 1; chondroblastoma, 2; clear cell sarcoma, 1; and Ewing's sarcoma, 3.

Conclusion: The rarity of talus tumors and anatomical complexity of the talus itself poses diagnostic challenges and management. Thus both the radiologist and orthopaedic oncologist need to be aware of these lesions to provide a timely diagnosis and management.
