Long-Term Outcome of Giant Olfactory Groove Meningioma Resection

Background: Olfactory groove meningiomas (OGMs) account for 13% of all intracranial meningiomas and known to be slow-growing tumors. Usually, it will not be detected until reaching a large size and its symptoms varies from headache to personality changes, insomnia, and visual deterioration. A little is known about the long-term outcomes of Giant olfactory groove meningioma patients.
Study Design and Methods: This is A retrospective study conducted in large medical city. We included all the patients with Giant olfactory meningioma (≥6 cm of diameter) and were treated surgically during a period of 22 years (Jan 2000–Dec 2022). Long-term outcome including visual status, recurrence and functional status were collected. Multivariable logistic regression was used to identify predictors of functional outcome.
Results: 32 patients met the criteria and included in our study. The mean age of our patient was 55.8 years and most of them female (71%). The mean follow up was 62 months. Most of our giant OGMs reported as WHO grad 1 (84.4%). The improvement of visual acuity and visual field deficits was observed in 19 patients. Recurrence was observed radiologically in nine patients (28.1%) at mean of 56 months follow up. Only 3 required reoperations. One patient had brain abscess following rhinorrhea and required surgery. Multivariable analysis identified age, Simpson grade of excision, and WHO grading of tumor were the factors that significantly affected the recurrence rate.
Conclusions: We are reporting the largest study on the long-term outcomes among giant olfactory groove meningioma patients. Our study showed that visual deficits will improve in most these patients after the surgery. The postoperative outcome was strongly predicted by factor like patient age, extend of resection and histological status. Building a new predictive scale based on these parameters appears to strongly predict the OGMs outcome.
