While endoscopic endonasal access for sellar lesions has gained popularity, it often requires a coordinated approach between neurosurgeons and otolaryngologists. In this technical presentation, we give an overview of the direct endonasal approach to the sella for lesions not requiring anterior or lateral extended exposure of the skull base. The purpose of this video is to teach neurosurgeons how to expose the sella and perform a basic reconstruction to expand the neurosurgeon's procedural repertoire.
