Keyhole Retractorless Retrosigmoid Approach for a Large Cystic Vestibular Schwannoma

We present a surgical video of a keyhole retrosigmoid retractorless approach for resection of a giant cystic vestibular schwannoma. This is a case of a 34-year-old male who presents with approximately 3 years history of progressive left hearing loss, left ear tinnitus and worsening vertigo. Although cystic vestibular schwannomas tend to be more adherent to the neural structures, we performed a small retrosigmoid craniectomy with the patient in supine position and achieved gross total resection.
