A 37-year-old female presented with 2 months of headache, blurry vision, and a partial CN3 palsy. MRI demonstrated a right-sided heterogeneously enhancing mass causing compression at the right pons, midbrain, and peduncle. Decision was made to take the patient for a retrosigmoid trans-tentorial approach (RSTTA) for resection of the lesion, which was demonstrated to be a trochlear schwannoma. The patient did well postoperatively with improvement in her CN 3 palsy.
