“Cold Steel”: Endoscopic Trans-clival and Nasopharyngeal Approach for Removal of a Penetrating Blow Dart and Skull Base Reconstruction

A 7-year-old female presented after a blow-dart penetrated her nasal ala and lodged in the nasal cavity and skull base. CT/CTA confirmed a 9.6-cm foreign body breached the clival dura at the craniovertebral junction with the tip of the projectile abutting the vertebral artery.

We performed an endoscopic trans-clival and nasopharyngeal approach to remove the blow-dart and complete the skull base reconstruction. Patient did well and was discharged home after 5 days with no neurologic deficit.
