A Case of Balancing Act: Optimizing Oncofunctional Outcomes in Endoscopic Craniopharyngioma Resection

We report on an 80-year-old male with progressive visual decline undergoing resection of a large suprasellar retro-infundibular craniopharyngioma via an expanded endoscopic endonasal approach. The video emphasizes critical intraoperative decisions essential for optimal outcomes and meticulous preservation of vital structures, including the stalk, superior hypophyseal arteries, and the thin mantle at the floor of the third ventricle. Close attention was paid to the tumor's proximity to the basilar, posterior cerebral, and superior cerebellar arteries.
