Tetanus after unsupervised fall

Introduction We describe the case of a 5-year-old boy who presented a few days after an unobserved fall with lockjaw and altered gait. The boy had never been immunized against tetanus.

Methods The unusual course of disease of the child is presented. Due to the anti-vaccination debates, this is a current and improtant topic.

Results The MRI showed contrast accumulation in the left frontobasal region. With increasing laryngospasm, protective intubation had to be performed. A skull base exploration with locating ans removal of wood was performed in the ENT operating room. Due to respiratory insufficiency, the boy was invasively ventilated for several days in the pediatric intensive care unit. He received intravenous antibiotic therapy. After several weeks, the boy could be discharged to rehabilitation with few residuals.

Discussion Despite the rarity of the clinical picture, a careful anamnesis on vaccination and, if necessary, a tetanus booster must be considered for every injury pattern.
