OP-5 Antiseizure Medication Adherence: Understanding the Factors That Influence, Our Role and Tools to Measure

Antiseizure medications (ASMs) are primary line of treatment for seizure disorders. Medication nonadherence is reported to have poor outcomes and increased health care costs. Our previous study in mixed population showed significant influence of socioeconomic status on adherence to ASMs. In the present study factors influencing and extent of adherence to ASMs was investigated in corporate hospital.
Data were collected from 322 PWE for demography, socioeconomic status, seizures, co-morbidities, ASMs, and ADRs over 2 years and analyzed using RV3.6.2. Adherence to ASMs was measured using calendar and Morisky Medication Adherence Scale (MMAS-4).
Mean age of PWE 29.576 ± 17.57yrs, no difference in gender distribution and marital status. 48.38% had generalized motor tonic clonic, 26.40% focal motor unaware, and 8% focal motor aware seizures. 98% had no seizure diary. ASM monotherapy was 57%, dual 31% and remaining three/four. ASMs used were Levetiracetam 51.24%, Valproate 22.67%, and other ASMs. MMAS-4 scores were 67.42%. CNS-ADRs were 49.01%. Univariate logistic regression for nonadherence revealed significant association of ADRs, stress and lack of sleep with odds ratio 2.62 (1.59–4.33), 2.2 (1.3–3.8), and 1.84 (1.06–3.19) respectively with ADR and stress as two primary factors for nonadherence [adjusted odds ratio 2.31 (1.38–3.87); 1.81 (1.03–3.18), respectively].
Study concludes ADRs, stress and forgetfulness contributed significantly to nonadherence to ASMs. We suggest need for using additional robust methods for measuring multiple medication adherences, tools to identify modifiable factors, tips to improve and formulate appropriate interventions to improve ASMs adherence among PWE of varied population to achieve optimal seizure control. These will be presented briefly.
