Case presentation: Elderly patient, 93 years old, diagnosed with clear cell renal carcinoma, who underwent right renal nephrectomy in 2012, at an early stage. In 2014, he had a local relapse and was treated with three-dimensional conformal external radiotherapy, total prescribed dose of 35Gy in 5 fractions. There was complete disappearance of the lesion after 3 years. Patient remains asymptomatic and has no signs of sequelae. Discussion: Clear cell renal carcinoma is the most common variant of renal tumors and is historically considered a radioresistant tumor. About 40% of patients die due to the progression of this disease, making this tumor the most lethal urological malignant lesion. The most common metastases are lungs and bones, and relapse event after radical nephrectomy is a rare event, occurring in less than 2% of cases. However, with the emergence of hypofractionation, high doses performed in small volumes of treatment, allowed a better management of the tumor, thus bringing better Results: final comments Local relapse of clear cell carcinoma is a rare event. In this case, surgical treatment and chemotherapy were refused by patient and family members. Hypofractionated radiotherapy was considered as an alternative and has an excellent response. It was a safe approach and provided effective local control.
