After an initial ICA exposure, the Sylvian fissure was superficially opened. Then the ICA bifurcation was followed distally to expose the MCA bifurcation. A temporary clip was applied on M1. Three aneurysms were identified on the superior trunk. First, the two distal aneurysms were clipped parallel to M1. Then, the operative microscope was rotated to clip the proximal aneurysm in a perpendicular plane in respect to M1.
