Microsurgical Clipping of a Large Ophthalmic Artery Aneurysm with Use of Endovascular Balloon Guide for Proximal Control and Aspiration Decompression with Prophylactic STA-MCA Bypass
A 35-year-old female referred for decreased right eye vision was found to have a large 1.1 × 1.0 × 1.6 cm right carotid-ophthalmic artery aneurysm. She underwent an angiogram and balloon test occlusion. Given compression of the optic nerve, decision was made to pursue microsurgical clipping with endovascular proximal control and decompression using a balloon guide with aspiration in the right cervical ICA and prophylactic right STA-MCA bypass. Patient tolerated the procedure well and her vision improved.
