Internal Carotid Artery to M2 Bypass and Clip Trapping for Giant M1 Aneurysm

We present the case of an 11-year-old girl with two giant aneurysms of the M1 segment of the middle cerebral artery. We perform an ICA to M2 bypass using a saphenous vein graft, followed by clip trapping of the diseased M1 segment. We review preoperative planning, anatomic considerations, and surgical technique.
