We present a case of progressive cavernous sinus meningioma causing severe ICA stenosis in a patient who failed a balloon occlusion test but wanted aggressive treatment. We performed a left orbitozygomatic craniotomy for internal maxillary artery to M2 bypass via a radial artery interposition graft and coiling of the petrous ICA to allow later sacrifice during tumor resection.
