An ideal neuromonitoring method should include a real-time alert system, quantitative evaluation, and functional assessment. This video demonstrates continuous abducens nerve monitoring in cerebellopontine angle epidermoid cyst surgery through the anterior transpetrosal approach. Using a stimulating electrode placed on the proximal abducens nerve, the evoked extraocular muscle electromyograms were continuously monitored by direct 1-Hz stimulation throughout the total resection. The patient had no postoperative neurological deficit.
