Anterior Transpetrosal Approach for Recurrent Pediatric Ependymoma in Cerebellopontine Angle

This video demonstrates an anterior petrosal approach for a pediatric ependymoma that recurred in the cerebellopontine angle. A 5-year-old male previously underwent a suboccipital craniotomy for a large fourth ventricular ependymoma, followed by chemoradiation. There was subsequently a small recurrence in the cerebellopontine angle for which he was recommended surgery. We elected for a middle fossa transpetrosal approach for a direct extradural trajectory to the lesion and to avoid his prior surgical site.
