Emergency Expanded Retrosigmoid Approach for Resection of Multiply and Acutely Rebleeding Pontine Cavernoma with a Large DVA

This is a case of a 19-year-old female with a very aggressive brain stem cavernoma. She presented in the peripheral hospital with headache, vertigo and photophobia, and evidence of bleeding. Within the next 2 days, she had two more episodes of bleeding resulting in her becoming comatose and developing cranial nerve deficits. She underwent emergency right extended retrosigmoid approach, splitting of horizontal fissure, and resection of the cavernoma through the middle cerebellar peduncle.
