This video details the endoscopic endonasal transsphenoidal resection of a large sellar/suprasellar mass in a 56-year-old male with bilateral blindness. The patient, with a history of renal cell carcinoma, presented with a highly vascular and heterogeneous lesion. Postoperative pathology confirmed the coexistence of metastatic renal cell carcinoma and a gonadotroph pituitary neuroendocrine tumor (PitNET). The video demonstrates the surgical technique, challenges faced due to the lesion's size and high vascularity, and postoperative outcomes.
